Your animal experience fee gives you:

Up-close, in-person access to the animals
The opportunity to pet and interact with them
One cup of feed so you can feed our amazing animals

Before entering the animal area, please complete the following two steps:

Complete the information below and agree to the liability waiver.
Submit the animal experience fee of $3 per person (including children) by either:
- Placing cash in the cash donation container located at the bar, or
- Sending payment via Zelle to (216) 650-9877

If paying with Zelle, you must show the bartender your payment confirmation before access to the animals will be granted. If paying with cash, you must also check in with the bartender after placing your donation in the cash donation container.

Please note:

The liability waiver is a one-time form and does not need to be completed again if you have already submitted it during a previous visit.
The animal experience fee must still be paid for each visit to the animal area.

All children under 18 MUST be

accompanied by an adult.

All proceeds go directly toward the

care, feeding, and upkeep of the animals.

Visitor Liability Waiver and Release Form

I, the undersigned participant (or parent/guardian of the minor participant), acknowledge that I have that I have voluntarily chosen to visit and interact with animals at Memphis Avenue Animal Sanctuary.

Assumption of Risk

I understand and accept all risks associated with interacting with animals, including but not limited to bites scratches, pecks, kicks, allergic reactions, zoonotic diseases, and other unforeseen incidents that may occur during contact with pigs, goats, ducks, chickens, and other animals on the premises.

Feeding Restrictions

I will only feed animals with food provided or approved by the sanctuary. I understand that bringing outside food for the animals is strictly prohibited as it may cause harm to them.

Supervision of Minors

Adults are responsible for directly supervising children at all times. I will ensure children in my care follow all rules, including gentle touching, quite voices, and proper hand washing after contact with animals. I understand that failure to follow these guidelines may result in removal from the property without refund.

Health Disclosure

I confirm that I (or my child) have no known severe allergies to animals, hay, straw, feed, or other elements present at the sanctuary that would place me at high risk during this visit.

Open Wounds

I confirm that I (or my child) do not have open cuts, wounds, or skin infections that might increase the risk of infection from animal contact.

Liability Release

In consideration of being permitted to enter and participate in activities at Memphis Avenue Animal Sanctuary, I, on behalf of myself, my heir, assigns, and next of kin, waive, release, and discharge the sanctuary, its owners, operators, employees, volunteers, and agents from any and all liability for injuries, illness, death, or property damage that may occur during normal animal interactions and related activities, excluding cases of gross negligence or intentional misconduct.

Duration of Agreement

I understand and agree that this Waiver and Release shall remain in full force and effect fir the entire duration of my presence at Memphis Avenue Animal Sanctuary, and shall also apply to any future visits or participation in activities at the sanctuary, unless and until revoked in writing by Memphis Avenue Animal Sanctuary.

By signing below, I acknowledge that I have read, understood, and agree to the terms on this page. I am at least 18 years of age and legally competent to sign this Waiver and Release, or I am the parent/guardian of the minor listed below.
